COPD Care – HGE Health Partnership
At Riverside Medical Group, we care about your health and want to help you maintain and improve your quality of life. That’s why we’ve partnered with HGE Health to offer a new HGE COPD Care Program to help you take control of your COPD symptoms.
How HGE COPD Care Works
Once enrolled, you’ll report your symptoms daily through the HGE Health website, app, or by calling the HGE Contact Center. You don’t need to be tech savvy to participate and patients generally spend less than two minutes per day on reporting.
If your symptoms differ from what’s typical for you, called your baseline, a nurse at the HGE Contact Center will be notified to review your information. If necessary, your doctor will be notified.
That’s it! It’s that simple.

Daily reporting of your symptoms will give you access to same-day feedback from your doctor, helping to avoid unnecessary office visits and trips to the ER. No more wondering if you should make an appointment, head to the hospital, or just wait and see how you feel later.
You’ll know when your symptoms require action and what you should do, giving you peace of mind and more power over your COPD. This keeps you healthier and safer. When your condition is better managed, it usually results in lower health care costs, too.
Succeed in Managing COPD
Nearly all patients enrolled in the program indicate that daily reporting of symptoms helps them better understand and manage their COPD. More than two-thirds of patients find they experience significant control of their symptoms after one month of participation. Most patients find this monitoring and care has helped them avoid the emergency room or urgent care facility and has given them an overall improved quality of life.
What Is the Cost of HGE COPD Care?
Your insurance company will be billed directly for your use of this service. Your insurance may cover the entire cost of the program. In some cases, you may be responsible for a small copayment. We can help you identify what your out-of-pocket costs might be.
